Job summary

Your Health Organization in Marietta, GA is seeking a Nurse Practitioner to deliver in-home patient care across our service area. You will evaluate high-risk patients, develop individualized care plans, and coordinate with an interdisciplinary team to ensure optimal outcomes.

This is a full-time salary-based role with the opportunity to earn a monthly bonus. The provider will perform visits in designated service areas, including homes, clinics, and telehealth, and will collaborate to deliver

Qualifications

Nurse Practitioner license in good standing (state of employment).
Experience in home health, clinics, or telehealth settings preferred.
Strong communication and collaboration within interdisciplinary teams.

Responsibilities

Evaluate high-risk patients and develop individualized care plans.
Coordinate care activities with interdisciplinary team members to ensure continuity of care.
Communicate with case managers, clinicians, pharmacists, and others to coordinate care.
Participate in team meetings to discuss patient progress and plans.
Document care activities accurately for compliance.
Provide patient education and resources for self-management.
Support patients and families in understanding options and promoting engagement.
