Job summary

Your Health is seeking a Nurse Practitioner to service patients in the Athens, Georgia area. You will provide care in patients' homes and facilities, collaborating with a multidisciplinary team to manage high-risk patients and coordinate care plans.

This full-time, salary-based role requires reliable transportation and offers a monthly bonus. The position covers Athens, Elberton and Hartwell with a Monday–Friday schedule and travel between offices and facilities.

Qualifications

• Must hold NP/PA/Physician license in the employing state.
• Experience delivering patient care, preferably in home health, clinic, facility, telehealth or specialty care.

Responsibilities

• Evaluate high-risk patients and develop care plans.
• Coordinate care with interdisciplinary team for continuity and outcomes.
• Communicate with case managers, coordinators, social workers, and pharmacists to align patient care.
• Meet staff efficiency expectations while providing high-quality care in homes, facilities, telehealth, clinics, and specialties.
