Job summary

Optum Home & Community Care is expanding its HouseCalls program in Missouri. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct in-home health assessments for Medicare Advantage and other plan members, focusing on education, gap closure, and coordination with PCPs.

This non-prescribing, field-based role requires no on-call, weekends, or holidays and emphasizes patient education, equity, and access to care with flexible scheduling and a strong support

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted Missouri license by start date, without disciplinary actions.
• Reliable transportation to complete home visits and access to a vehicle.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and coordinate with primary care providers.
• Educate members on disease processes and medications, and address social determinants of health.
• Identify urgent situations and intervene or refer as needed.
