Job summary

Optum Home & Community Care is seeking a Nurse Practitioner to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes.

You’ll operate without on-call, weekends, or holidays, and will receive a comprehensive benefits package and competitive total cash compensation tied to performance.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in Texas (or ability to obtain by start date) without disciplinary actions or practice limitations.
• Active and unrestricted driver's license with access to reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments (sitting/standing/kneeling).
• If applicable, you must obtain licensing board approval within specified timelines after hire.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Recognize urgent or emergent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
