Optum Home & Community Care, part of the Optum family, conducts in-home health assessments for Medicare Advantage and other plan members. This role, as a HouseCalls Advanced Practice Clinician (NP/PA), involves education, gap closure, and collaboration with primary care providers to improve health outcomes.

The position offers flexible scheduling (8am–8pm over 7 days), no on-call, no weekends or holidays, and a sign-on/loan repayment option with language-diverse compensation opportunities.

Qualificações

• Active ANCC or AANP certification for NPs or NCCPA for PAs.
• Active, unrestricted state licensure (or able to obtain by start date).
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments during assessments.

Responsabilidades

• Conduct in-home assessments for Medicare Advantage and other plan members.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
