Job summary

Optum Home & Community Care in Indiana is seeking a HouseCalls Advanced Practice Clinician (NP/PA) to perform annual in-home health assessments for Medicare Advantage and other plan members. This field-based role focuses on education, gap closure, and collaboration with PCPs.

Required: NP/PA certification, active state licensure, driving license with reliable transportation, and ability to navigate client homes. Sign-on bonus details apply; flexible 8am–8pm schedule with no weekends or on-call.

Qualifications

• ANCC or AANP Family/Adult/Geriatric/AGPC certification.
• Current NCCPA certification and state licensure.
• Active, unrestricted state licensure with start-date readiness.
• Active and unrestricted driver's license.
• Access to reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.
• Eligibility process for independent practice where applicable.

Responsibilities

• Conduct comprehensive in-home assessments including history, meds, vitals, and exam.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close care gaps.
• Recognize urgent situations and intervene appropriately.
• Educate members on disease processes, meds, and adherence.
• Address social determinants of health and provide referrals.
