Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP/PA) to perform comprehensive in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, care coordination, and gap closure.

You will conduct visits between 8am–8pm across a 7-day schedule with flexible hours, no on-call, weekends, or holidays required. A sign-on bonus and potential language-based incentives are included.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• For PAs: Current NCCPA certification and state licensure.
• Active, unrestricted licensure in the practicing state without disciplinary actions.
• Active and unrestricted driver's license with reliable transportation.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Perform in-home health assessments for Medicare Advantage and other plan members.
• Review past medical history, medications, vital signs, and conduct exams.
• Document diagnoses for care management and coordinate with PCPs.
• Identify urgent situations and intervene appropriately.
• Educate members on diseases, medications and adherence.
• Address social determinants of health and arrange referrals.
