Qualifications

• Bachelor's degree in nursing from NLN-accredited program.
• Current Maryland RN license and Nurse Practitioner certification.
• Valid Maryland driver's license.
• Master's degree in public health or nursing specialty preferred.
• Successful completion of NP program and state certification in Maryland.

Responsibilities

• Collaborate with physicians and other health professionals to design and implement comprehensive care plans for patients at risk for or managing chronic conditions such as diabetes, obesity, cardiovascular disease, and hypertension.
• Provide health teaching, behavioral counseling and guidance to patients and families; conduct behavioral health assessments.
• Perform health screenings (blood pressure, glucose, cholesterol, BMI) and document outcomes.
• Maintain accurate documentation of therapy sessions, treatment plans, and patient progress.
• Assist with administering chronic disease programs and Family Healthy Weight initiative as needed.
• Train staff on standardized protocols to improve care plan development and chronic disease management.
