Job summary

Prince George’s County Government seeks a Nurse Practitioner I/II to provide advanced nursing care in the Health and Wellness Division. You will deliver preventive and disease management services to individuals, families, and groups, addressing at-risk populations and chronic conditions such as obesity and diabetes.

You will educate patients, conduct behavioral health assessments, and collaborate with a multidisciplinary team to implement evidence-based plans.

Qualifications

Graduation from a National League for Nursing accredited college or university or an equivalent program as determined by Maryland State Board of Nursing; with a Bachelor's Degree in nursing.
Current license to practice as a Registered Nurse.
Nurse Practitioner program completion and certification in Maryland.
Master's Degree in public health or related field may substitute for the Master's degree requirement.
Possession of a valid driver’s license.
Equivalent education/experience may substitute for master's degree but not for NP program.
EPIC software experience preferred.

Responsibilities

Collaborate with pediatricians, dietitians, psychologists and other healthcare professionals to design evidence-based care plans for patients at risk or managing chronic conditions.
Assist with recruitment, community organizations, pharmacies and eligible participants for chronic disease programs.
Develop individualized educational plans for patients and families focusing on healthy lifestyles, nutrition and activity.
Maintain accurate documentation of therapy sessions, treatment plans and patient progress.
Assist Program Chief with administering chronic disease programs emphasizing diabetes prevention and obesity management.
Implement Family Healthy Weight Program (Family-based Behavioral Treatment).
Train staff on standardized protocols to improve care planning and outcomes.
Conduct health screenings per established protocols (BP, glucose, cholesterol, BMI).
