UnitedHealth Group is seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to conduct annual in-home health assessments for Medicare Advantage and other plan members. The role emphasizes improving health outcomes through direct patient education and collaboration with primary care providers.

This position offers flexible scheduling, with no on-call duties, no weekends, and no holidays required. Ideal candidates will possess relevant certifications and a commitment to helping patients navigate the healthcare system efficiently.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric or Adult-Gerontology Primary Care.
• Active driver's license and reliable transportation for home visits.
• 1+ years of clinical experience preferred.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage members.
• Identify and document diagnoses for care management.
• Educate members on disease processes and compliance.
