Job summary

UnitedHealth Group is seeking an in-home HouseCalls Advanced Practice Clinician (NP/PA) to conduct comprehensive home health assessments for Medicare Advantage members. The role emphasizes education, care-gap closure, and coordination with PCPs.

The position offers flexible scheduling, no on-call or weekend work, and a field-based workflow that brings care directly to patients in their homes.

Qualifications

• Active NP certification (ANCC/AANP) or PA certification (NCCPA) required.
• Active, unrestricted state licensure to practice in the start state.
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments during assessments.

Responsibilities

• Perform in-home health assessments for Medicare Advantage and other members.
• Document diagnoses for care management and treatment planning.
• Share findings with primary care providers to close care gaps.
• Identify urgent situations and intervene appropriately.
• Educate members on disease processes, medications and adherence.
• Address social determinants of health and arrange referrals as needed.
