Qualifications

• Candidates must have active ANCC or AANP certification.
• Must have active, unrestricted licensure in Colorado.
• Ability to transport equipment and navigate home environments.

Responsibilities

• Conduct comprehensive in-home assessments.
• Identify and document diagnoses for care management.
• Communicate findings to members' PCPs.

Skills

Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care

Ability to navigate varied home environments

Proven communication skills with geriatric or Medicare populations
