Job summary

Upperline Health is seeking a Nurse Practitioner to provide clinic-based care in our Roswell, GA Clinic. In collaboration with Upperline’s Physician team, the APP develops complex plans of care, provides diagnostics and treatments in clinic, home, or telehealth, and educates patients and families on medication usage and crisis prevention.

Ideal candidates have Primary Care/Internal Medicine or geriatric/home-care experience, a compassionate approach, and thrive in a fast-growing

Qualifications

• Master’s degree in nursing required.
• Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner required.
• Basic Life Support (BLS) certification is required.
• 6 months nurse practitioner experience in Primary Care/Internal Medicine.
• Value-based care experience preferred.
• Possession of DEA registration or eligibility.
• Must be comfortable providing care in patients’ homes.
• Experience with AthenaHealth EMR is highly preferred.
• Excellent oral and written communication skills to ensure effective interactions.

Responsibilities

• Develop and implement clinical plans of care for adult patients with chronic conditions.
• Gather patient history, exam, and diagnostics in clinic, telehealth, or home.
• Develop and implement treatment plans aligned with patient goals.
• Collaborate with interdisciplinary team to meet physical and psychosocial needs.
• Provide compassionate care for complex patients and families.
• Educate patients and families about medications, nutrition, and crisis prevention.
• Coordinate care with community providers and Upperline clinicians.
• Maintain accurate medical records and documentation per regulations.
