Job summary
Upperline Health Inc. is seeking a Nurse Practitioner to provide clinic-based and home-based care in the Columbus/Seymour, IN area.
The APP will collaborate with physicians to develop and implement complex care plans for adults with chronic conditions, offering diagnostics, treatments, and patient education in person or via telehealth. The ideal candidate will hold a Master’s in Nursing, be board certified as Adult, Adult-Geriatric or Family NP, and be comfortable working in a value-based care
Qualifications
- Master’s degree in nursing required.
- Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner required.
- Value-based care experience preferred.
- DEA registration or eligibility required.
- BLS certification required.
- Comfort providing care in patients' homes.
- Experience with EMR; AthenaHealth EMR preferred.
Responsibilities
- Develop and implement clinical plans of care for adults with chronic conditions in clinic or home settings.
- Gather patient history, perform physical exams, and order diagnostics in home or clinic settings.
- Collaborate with interdisciplinary team to meet physical and psychosocial needs of patients.
- Educate patients and families on medications, diet, nutrition, and crisis prevention.
- Prescribe medications as permitted by state nursing board.
- Maintain accurate medical records and documentation per regulations.
