Job summary

Nectar Locums LLC Seaside, OR is seeking a PA or NP with geriatric experience for outpatient clinic coverage starting in November.

The assignment combines in-clinic care with community visits to adults 55+, often traveling to assisted living, SNFs or residences. You will work with an 11-person interdisciplinary team, manage visits in an EMR, and follow a standard 8am–5pm schedule with 8 hours per day.

Qualifications

• Geriatrics experience required for majority patient population.
• Active medical license (PA/NP) in good standing.
• DEA license required for prescribing.
• BLS certification required.

Responsibilities

• Deliver outpatient services in clinic and community settings.
• Travel to patient residences (assisted living, SNFs, private homes) with nursing support.
• Collaborate with an 11-person interdisciplinary team and document in EMR.

• Provider Min # Shifts Required: 2 weeks per month minimum (ideally would like a provider who is full time)
• Guaranteed Hours per Day: 8
• Hours Worked for OT to apply: 9

Additional Details

This site has a combination of in-clinic and community patients who are seen in their home. For off-site visits, this will involve driving to the patient’s residence which can be Assisted living, SNF, or personal residence where usually with a nurse from our team will join the provider for the visit. We serve exclusively adults over the age of 55 as a PACE program (Program of All Inclusive Care for the Elderly) which means you’ll be working with a 11 person interdisciplinary team. Most patient management is handled through the EMR with other team members coordinating visits and care in the community.
