Job summary

BAS Healthcare, Inc. in Portland, OR, seeks an Advanced Practice Nurse Practitioner (Gerontology) for an outpatient, clinic-only role with no on-call requirements. Monday–Friday, 8:00 am–5:00 pm, with potential 3–4 day weeks. Start date November 2026; full-time availability preferred.

This facility operates as a PACE program (Program of All Inclusive Care for the Elderly), dedicated exclusively to serving adults over the age of 55. The program features a collaborative, interdisciplinary care model with an 11-person team providing both in-clinic and community-based care, with most patient management coordinated through an EMR system.

Job Details:

• Outpatient clinic-only position with no call requirements
• Monday through Friday schedule, with consideration given to 3-4 day per week availability
• Shift hours from 8:00am to 5:00pm with 8 guaranteed hours per day
• Start date of November 2026, ongoing need
• Minimum of 2 weeks per month required; full-time availability strongly preferred
• See 4-6 patient encounters per shift
• Geriatric experience required, as the patient population is exclusively adults over the age of 55
• Provide both in-clinic visits and off-site community visits to patient residences including assisted living facilities, SNFs, and personal residences
• Off-site visits will include driving to patient locations, accompanied by a nurse from the care team
• Work collaboratively within an 11-person interdisciplinary care team
• Active Oregon license required; licensed in OR only
• Board certified or truly board eligible required
• Active DEA required
• BLS certification required
• Current CV must be uploaded with bid submission

Compensation:
