Qualifications

• Active Nurse Practitioner certification with geriatrics focus.

• Compassionate, patient‑centered approach to care.
• Proficient in EHR documentation.

Responsibilities

• Conduct comprehensive physical examinations and manage acute and chronic geriatric illnesses.
• Develop and implement individualized care plans addressing medical, physical, and psychosocial needs.
• Prescribe medications, order/interpret diagnostics, and perform procedures within scope.
• Collaborate with physicians, nurses, social workers, and therapists for coordinated care.
• Educate residents and families about health conditions and treatment options.
• Advocate for residents’ wellbeing and ensure appropriate care.
• Document patient care accurately in the EHR.
