Job summary

GuideStar Eldercare is seeking a Nurse Practitioner to address dementia and neurocognitive disorders in nursing homes and assisted living facilities. You will work autonomously with regional support, travel to assigned facilities daily, and provide comprehensive evaluations and cognitive testing.

You will document services, collaborate with a multidisciplinary team, and build strong relationships with patients, families, and facilities. Leadership supports scheduling, billing, and compliance.

Qualifications

• Proof of Nurse Practitioner licensure.
• Ability to work as part of an interdisciplinary treatment team.
• Good time management skills.
• Able to function autonomously with regional Area Manager support.
• Pass pre-employment background check and drug screening.
• Complete credentialing process.

Responsibilities

• Travel daily to assigned facilities from your home.
• Provide initial evaluations, continuing therapy services, and cognitive testing.
• Document the need and extent of services for each patient.
• Create and maintain positive professional relationships with client facilities, patients, and their families.
