Job summary

GuideStar Eldercare is seeking a Nurse Practitioner to address the needs of residents with dementia and other neurocognitive disorders in nursing homes and assisted living facilities. You will work with clinical leadership, enjoy autonomy with a defined caseload, and receive administrative support for scheduling, coding, and billing.

The role includes travel to facilities from home, initial evaluations, therapy continuation, cognitive testing, and documentation of services.

Qualifications

• Minnesota state licensure as Nurse Practitioner.
• Pass pre-employment background check and drug screening.
• Complete credentialing process.
• All GuideStar employees must be vaccinated against COVID-19 and flu.

Responsibilities

• Travel daily to assigned facilities from home.
• Provide initial evaluations, continuing therapy services, and cognitive testing.
• Document the need and extent of services for each patient.
• Build and maintain positive professional relationships with client facilities, patients, and families.
