AlignMed Partners is seeking a full-time Advanced Practice Provider (NP or PA) to deliver primary and urgent care at Laureldale Skilled Nursing and Rehabilitation Center in Laureldale, PA. The role requires at least 1 year of provider experience and licensure in respective practice states.

The position offers a base salary of $114,400–$124,800 per year plus a no-cap incentive bonus program. Relocation support and comprehensive benefits are provided.

Qualifications

• Graduate from an accredited nursing program or PA program.
• Nurse Practitioner with Master's degree or PA with ARC-PA accreditation and PA licensure.
• Two years of adult/geriatric clinical experience preferred.

Responsibilities

• Provide primary and urgent care to nursing center residents.
• Collaborate with Medical Director and Center Leadership.
• Support quality care and documentation in collaboration with the care team.
