Job summary

AlignMed Partners is seeking a full-time Advanced Practice Provider (NP or PA) for our Quakertown, PA center. You will deliver primary and urgent care to skilled nursing residents, collaborating with leadership and medical staff to meet center goals.

The role requires NP or PA credentials, with a focus on older adult or family practice, and at least two years of geriatric experience. Expect a base salary of $120,640–$124,800 with an incentive program and no on-call weekends.

Qualifications

• Graduate from an accredited nursing program and RN licensed; NP with Master's in Nursing and Older Adult/Family NP concentration.
• PA graduate from ARC-PA accredited program; valid PA licensure and NCCPA certification.

Responsibilities

• Provide primary and urgent care to nursing center residents.
• Collaborate with Center Leadership and Medical Director to meet patient care goals.
• Support clinical leadership and participate in program initiatives.
