Job summary

Brio Living Services is seeking a Nurse Practitioner to join Thome PACE in Jackson, MI, focusing on medically complex older adults within an interdisciplinary team. You will design care plans and coordinate with nursing, social work, therapy, pharmacy, and home care to ensure patient-centered, resource-conscious care.

Qualifications

• Bachelor’s Degree in Nursing from an accredited college or university, with graduation from a certificate or accredited Nurse Practitioner program.
• A valid Nursing License and educational degree/diploma.
• Minimum of five years of combine experience as a Registered Nurse (RN) and Nurse Practitioner (NP).
• Current Michigan Controlled Substance License and DEA registration.
• Current specialty certification to practice as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP).
• Valid driver's license and reliable transportation

Responsibilities

• Perform health histories, physical exams, and patient education.
• Order, interpret, and follow-up on lab tests and results.
• Perform selected procedures and document care both orally and in writing.
• Assess, diagnose, treat, and coordinate participant care.
• Collaborate with ER physicians and hospital staff to manage care, prevent unnecessary hospitalizations, and facilitate expeditious discharge.
• Educate participants and families on preventive health maintenance, particularly for chronic conditions.
• Actively participate in family conferences to provide education and care planning.
• Actively engage in quality improvement (QI) processes to enhance care delivery.
