Job summary

Brio Living Services in Jackson, MI is seeking an experienced Physician Assistant to join the Thome PACE program. You will provide comprehensive medical care to medically complex older adults as part of an interdisciplinary team.

The role emphasizes patient-family education, evidence-based treatment plans, and collaboration with nursing, social work, pharmacy, and home care to optimize participant outcomes. Flexible schedule with strong benefits.

Qualifications

Graduate of an accredited Physician Assistant educational program.
Current Michigan Physician Assistant licensure required.
Current and unrestricted license to practice as a Physician Assistant in the state of Michigan.
Current Michigan Controlled Substance License and DEA registration.
Current national certification through the NCCPA.
Minimum of five years of clinical experience as a Physician Assistant, preferably in primary care, internal medicine, geriatrics, or related setting.

Responsibilities

Perform health histories, physical exams, and patient education.
Order, interpret, and follow-up on lab tests and results.
Perform selected procedures and document care both orally and in writing.
Assess, diagnose, treat, and coordinate participant care.
Coordinate care with ER physicians and hospital staff to manage care and prevent hospitalizations.
Actively participate in interdisciplinary team (IDT) meetings and care planning.
Complete clinical documentation, orders, assessments, referrals, follow up, and other provider responsibilities.
