Job summary

Optum HouseCalls is seeking a Nurse Practitioner or Physician Assistant to perform in-home health assessments for Medicare Advantage members in Arizona in a non-prescribing clinical role. You will focus on education, gap closure, and coordination with primary care providers as part of an interdisciplinary team.

Enjoy flexible scheduling (8am-8pm across a 7-day week) with 20–40 hours per week, no on-call, weekends, or holidays.

Qualifications

• Active NP/PA certification for primary care
• PA requires NCCPA cert and state licensure
• Arizona license active/unrestricted or able to obtain by start date

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage members
• Identify diagnoses for care management and treatment planning
• Communicate findings to PCPs to close gaps in care
• Recognize urgent situations and intervene appropriately
• Educate members on diseases, medications and adherence
• Address social determinants of health and provide referrals as needed
