Job summary

UnitedHealth Group's Optum HouseCalls team seeks an Advanced Practice Clinician (NP or PA) to perform annual in-home health assessments for Medicare Advantage members in Arizona. You will evaluate medical history, med reconciliation, vitals, and exam, document diagnoses, and coordinate with PCPs to close care gaps.

The role offers flexible scheduling (8am–8pm across 7 days; 20–40 hours weekly), no on-call, weekends, or holidays, and strong emphasis on education, prevention, and social

Qualifications

• NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• PAs: Current NCCPA certification and state licensure
• Active and unrestricted licensure in Arizona or eligibility
• Access to reliable transportation for home visits

Responsibilities

• Conduct in-home assessments including medical history review, med reconciliation, vitals, and exam
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to PCPs to address care gaps
• Recognize urgent situations and intervene as needed
• Educate members on disease processes, medications, and compliance
