UnitedHealth Group's Optum HouseCalls team is seeking a Nurse Practitioner or Physician Assistant to perform in-home health assessments for Medicare Advantage members across Oklahoma. This field-based role emphasizes education, gap closure, and collaboration with primary care physicians to improve health outcomes and access to care.

The position offers a balanced schedule with no on-call, weekends, or holidays.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in Oklahoma (or ability to obtain by start date) without disciplinary actions.
• Active and unrestricted driver's license; access to reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.
• If state allows independent practice, obtain licensure approval within required timelines.

Responsibilities

• Conduct comprehensive in-home assessments, including history, med reconciliation, vitals, and physical exam.
• Identify and document diagnoses for care management and planning.
• Communicate findings to PCPs to address gaps in care.
• Recognize urgent situations and intervene appropriately.
• Educate members on diseases, medications, and adherence; address social determinants of health.
