Job summary

UnitedHealth Group's Optum HouseCalls team seeks a Nurse Practitioner or Physician Assistant to perform in-home health assessments for Medicare Advantage members. This role focuses on education, gap closure, and collaboration with primary care providers to improve outcomes.

Responsibilities include comprehensive health evaluations, medication reconciliation, and addressing social determinants of health. Strong communication and licensure in the practicing state are required.

Qualifications

• Active, unrestricted NP/PA certification in appropriate specialty
• NCCPA certification and state licensure for PAs, with valid license in practicing state
• Active unrestricted licensure without disciplinary actions required

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other members
• Document diagnoses for care management and treatment planning
• Coordinate with PCPs to address care gaps and urgent situations
• Educate members on disease processes, medications, and adherence
• Address social determinants of health and provide referrals as needed
