Position Summary

We are seeking a licensed Nurse Practitioner (NP) to serve as a Clinical Chart Reviewer and Evaluator. The role involves conducting patient evaluations, reviewing clinical documentation to assess appropriateness of care, monitor treatment progress, and ensure alignment with our weight loss and musculoskeletal care programs. This position supports clinical quality, ensuring all care is evidence-based, effective, and properly documented.

Key Responsibilities

1. Conduct telehealth evaluations to assess weight management needs, musculoskeletal conditions, and overall clinical status.
2. Develop and implement individualized, evidence-based, and goal-oriented care plans aligned with program standards.
3. Review patient charts and medical records for accurate documentation of clinical findings, diagnoses, and treatment plans.
4. Identify gaps or inconsistencies in clinical documentation and patient care pathways.
5. Collaborate with internal teams to enhance quality assurance, program performance, and patient outcomes.
6. Maintain confidentiality and comply with HIPAA regulations.
7. Participate in initiatives to improve patient care and program efficiency.
8. Stay updated with current evidence-based practices and clinical guidelines.

Qualifications

• Graduate of an accredited Nurse Practitioner (NP) program.
• Active, unrestricted Nurse Practitioner license in the U.S.
• Minimum of 2 years clinical experience, preferably in primary care, internal medicine, or musculoskeletal/orthopedics.
• Experience with chart review, quality assurance, and care plan development.
• Strong attention to detail and clinical judgment.
• Familiarity with regulatory standards related to clinical documentation.
• Proficiency with Electronic Health Records (EHR) systems.
• Multistate licensure or NLC membership is highly desirable.
