Position Summary

We are seeking a licensed Nurse Practitioner (NP) to serve as a Clinical Chart Reviewer and Evaluator. The role involves conducting patient evaluations and reviewing clinical documentation to assess appropriateness of care, monitor treatment progress, and ensure alignment with our weight loss and musculoskeletal care programs. This position supports clinical quality, ensuring evidence-based, effective, and well-documented care.

Key Responsibilities:

• Conduct in-depth evaluations via telehealth to assess weight management, musculoskeletal conditions, and overall clinical status.
• Develop and complete individualized care plans based on evaluations, ensuring they are evidence-based and goal-oriented.
• Review patient charts and medical records for accurate reflection of clinical findings, diagnoses, and treatment plans.
• Identify gaps or inconsistencies in clinical documentation and patient care pathways.
• Collaborate with internal teams to improve quality assurance, program performance, and patient outcomes.
• Maintain confidentiality and comply with HIPAA regulations.
• Participate in initiatives to enhance patient care delivery and program performance.
• Stay updated with the latest evidence-based practices, clinical guidelines, and program updates.

Qualifications:

• Graduate of an accredited Nurse Practitioner (NP) program.
• Active, unrestricted Nurse Practitioner license in the U.S.
• Minimum of 2 years clinical experience as an NP, preferably in primary care, internal medicine, or musculoskeletal/orthopedic settings.
• Experience in chart review, quality assurance, clinical documentation improvement, and developing comprehensive care plans.
• Strong attention to detail and clinical judgment.
• Familiarity with regulatory requirements related to clinical documentation and care protocols.
• Ability to work independently remotely, with proficiency in Electronic Health Records (EHR) systems.
• Multi-state licensure or Nurse Licensure Compact (NLC) membership is highly desirable.
