Job summary

Duo Health is seeking a Nurse Practitioner to join our interdisciplinary care team focused on chronic kidney disease. You will provide comprehensive medical care, coordinate with physicians, social workers, care navigators, and community resources, and deliver care in homes, via telehealth, or by phone to a focused patient panel.

Qualifications

• Active NP/PA license (AZ or FL) and board certification.
• Credentialing per Duo Health requirements including background check.
• Ability to travel to patient homes with reliable transportation.
• Strong clinical assessment, communication and critical thinking skills.
• Ability to work independently while collaborating with a team.
• Experience delivering care in person and via telehealth.

Responsibilities

• Manage a focused panel of CKD patients within an interdisciplinary team.
• Conduct comprehensive health assessments, histories and exams.
• Diagnose and manage acute and chronic conditions within scope.
• Order, interpret and review labs and diagnostics.
• Prescribe and manage medications per state regulations.
• Develop individualized care plans addressing medical, behavioral, and social needs.
• Educate patients and caregivers on disease management and lifestyle changes.
• Coordinate care across primary care, specialists, hospitals and community resources.
• Participate in regular interdisciplinary care team meetings.
• Maintain patient confidentiality and document encounters accurately.
