As a Nurse Practitioner at Duo Health, you’ll be a core member of an interdisciplinary care team, providing comprehensive medical care and chronic disease management to a focused panel of patients living with CKD and other complex conditions. Your work will center on preventive care, chronic disease management, patient education, and care coordination all through a value-based, relationship-centered approach.
You’ll work closely with physicians, Social Workers, Care Navigators, specialists, and community resources to ensure care is coordinated, connected, and centered around the whole person. Patients are seen in their homes, virtually, or by phone, giving you the time and context to build meaningful relationships and gain a deeper understanding of each patient’s unique health needs.
