Travel to patient residences across Apache Junction area is required.

The role operates within a patient-centered, psycho-social care model and requires an Arizona APN license and strong communication skills.

Qualifications

• APN credential in AZ required.
• Background check required.
• Reliable transportation and ability to travel to patient homes.

Responsibilities

• Provide high-quality value-based care for CKD patients in home or telehealth settings.
• Collaborate with care team 0to coordinate comprehensive care.
• Perform history, physical exams, order labs and imaging as needed.
• Develop and implement patient-centered care plans and safety protocols.
