Job summary

Strive Health is seeking a Nurse Practitioner to lead longitudinal care for patients with chronic kidney disease, CHF, and related conditions. You will deliver in-home, clinic, and telehealth visits, collaborating with interdisciplinary teams to optimize outcomes and reduce hospitalizations.

This role emphasizes autonomy, advanced analytics, and patient-centered care, with a focus on reducing administrative burden and improving access across a hybrid work model.

Qualifications

• Master's degree in Nursing or similar field.
• 2+ years as an NP in cardiology or nephrology preferred.
• Active state NP licensure and DEA license or eligibility.
• Current BLS/CPR certification and reliable transportation.

Responsibilities

• Manage a defined patient panel with quality, cost and outcomes accountability.
• Conduct history taking, exams, tests, and care planning for CKD/CHF patients.
• Support transitions to renal therapies and end-of-life discussions.
• Document diagnoses for risk adjustment and care gaps.
• Provide care across home, telehealth, and clinic settings.
• Maintain board certification and continuing education requirements.
