Job summary

Strive Health is seeking a Clinical Lead Nurse Practitioner to drive comprehensive chronic kidney and heart failure care across in-home, telehealth, and clinic settings. You will leverage advanced analytics and collaborative teams to deliver high‑impact, patient‑centered care across diverse care settings.

You will coordinate with physicians, nurses, and care teams to reduce readmissions, manage complex cases, and engage patients and families in decision making.

Qualifications

• Master's degree in Nursing or similar field.
• 2+ years experience as a Family Nurse Practitioner (NP), Primary Care NP or equivalent with specialization in cardiology (CHF) or Nephrology (CKD).
• 2+ years experience with Electronic Health Records (EHR).
• Current state specific licensure.
• National Board Certification.
• Current DEA license or eligible to obtain within 90 days of hire.
• Current BLS or CPR Certification.
• Active driver's license and willingness to travel across an assigned region to meet patient needs.

Responsibilities

• Manages a defined patient panel with accountability for quality, cost and outcomes.
• Obtains patient history, performs physical exams, orders and interprets tests, and plans care.
• Incorporates CKD/ESKD and heart failure best practices into care plans, including transitions to renal therapies when needed.
• Documents diagnoses to support risk adjustment and care gaps.
• Presents cases and supports interdisciplinary rounds and meetings.
• Serves patients in multiple settings including home, telehealth, and clinics.
• Maintains board certification and ongoing education requirements.
• Provides in-person patient care and may involve travel.
