Job summary
Strive Health is looking for an experienced Nurse Practitioner to lead clinical care for patients with chronic conditions, delivering longitudinal, home-based and telehealth services. You will leverage advanced analytics and collaborative teams to ensure high-quality, patient-centered care across settings.
The role emphasizes autonomy, care coordination, and reducing hospitalizations, with a strong focus on CKD/ESKD and heart failure management.
Qualifications
- Master's degree in Nursing or equivalent.
- 2+ years as NP with cardiology (CHF) or nephrology (CKD).
- 2+ years with Electronic Health Records (EHR).
- Active state licensure.
- National Board Certification.
- DEA license or eligible within 90 days of hire.
- BLS or CPR certification.
- Reliable transportation for regional travel.
Responsibilities
- Manage a defined patient panel with focus on quality, cost and outcomes.
- Perform history, physicals, tests and formulate patient plans.
- Support CKD/ESKD, heart failure and transitions to renal therapies as needed.
- Document diagnoses for risk adjustment and care gaps.
- Present cases at rounds and interdisciplinary meetings.
- Provide care in home, telehealth, clinics or partner spaces.
- Maintain board certification and required CE credits.
- Travel within assigned region as needed.
