Job summary

Strive Health is seeking an experienced Nurse Practitioner to lead longitudinal, home-based and telehealth care for patients with CHF, CKD/ESKD, and other chronic conditions. You will work in a hybrid-remote capacity across multiple settings, leveraging advanced analytics and collaboration to deliver high-quality, patient-centered care.

The role emphasizes autonomy, reduced administrative burden, and a target annual salary between $112,000 and $140,000, with a 10% target bonus.

Qualifications

• Master’s degree in Nursing or similar field.
• 2+ years experience as a Family Nurse Practitioner (NP), Primary Care NP or equivalent with specialization in cardiology (CHF) or Nephrology (CKD).
• 2+ years experience with Electronic Health Records (EHR).
• Current state specific licensure.
• National Board Certification.
• Current DEA license or eligible to obtain within 90 days of hire.
• Current BLS or CPR Certification.
• Efficient and reliable transportation, including an active driver’s license, allowing for travel across an assigned region to meet patient needs.

Responsibilities

• Manages a defined patient panel with accountability for quality, cost and outcomes; collaborates with external providers and care teams.
• Obtains patient history, performs physical exams, orders and interprets tests; formulates a plan for short-term and longitudinal needs.
• Addresses CKD/ESKD and heart failure, supports transitions to renal replacement therapies or conservative care.
• Documents diagnoses to support risk adjustment (HCC coding) and addresses care gaps.
• Presents cases and provides clinical support for rounds and interdisciplinary meetings.
• Performs patient visits in home, telehealth, clinics, or partner spaces as applicable.
