Job summary
Strive Health is seeking a Clinical Lead Nurse Practitioner to drive interdisciplinary, patient-centered care for CHF, CKD/ESKD and related conditions. You’ll deliver longitudinal care in homes, clinics, and via telehealth, supported by advanced analytics and technology platforms.
You will lead a dedicated patient panel, partner with care teams, and maintain board certification while advancing evidence-based practices.
Qualifications
- Master’s degree in Nursing or similar field.
- 2+ years experience as a Family Nurse Practitioner (NP), Primary Care NP or equivalent with specialization in cardiology (CHF) or Nephrology (CKD).
- 2+ years experience with Electronic Health Records (EHR).
- Current state specific licensure.
- National Board Certification.
- Current DEA license or eligible to obtain within 90 days of hire.
- Current BLS or CPR Certification.
Responsibilities
- Manages a defined patient panel with accountability for quality, cost and outcomes.
- Obtains patient history, performs physical exam, orders and interprets diagnostic tests, and formulates a plan for individual patient needs.
- Documents diagnoses to support risk adjustment (HCC coding) and identify care gaps.
- Collaborates with care teams to align goals and care plans.
- Provides in-person and telehealth visits across multiple settings.
