Job summary

Strive Health seeks an experienced Nurse Practitioner to lead chronic care management for a defined patient panel in Indianapolis, delivering longitudinal home and telehealth visits within an interdisciplinary team.

You will leverage analytics and technology to optimize outcomes for CHF, CKD/ESKD and related conditions while focusing on patient‑centered goals and shared decision‑making.

Qualifications

• Master’s degree in Nursing or a related field.
• 2+ years as NP in cardiology (CHF) or nephrology.
• 2+ years of Electronic Health Records (EHR) experience.
• Current state NP licensure and NB Certification.
• DEA license or eligible within 90 days of hire.
• Current BLS/CPR certification.

Responsibilities

• Manages a defined patient panel with accountability for quality, cost and outcomes.
• Obtains patient history, performs exams, orders and interprets tests, and plans care.
• Ensures CKD/ESKD and heart failure care plans; supports transitions in renal therapies.
• Documents diagnoses to support risk adjustment and HEDIS improvements.
• Presents cases and supports rounds and interdisciplinary meetings.
• Provides in‑person, home, telehealth visits across settings including dialysis clinics.
• Maintains board certification and state CE requirements.
• Adheres to Strive’s documentation policy.
• Provides in‑person care including mobility tasks.
