Job summary

Circadia Health is seeking a Nurse Practitioner to join on-site in the Detroit metro area. You will see patients first, act on predictive health signals in real time, and coach facility teams to turn data into timely interventions that reduce preventable readmissions.

You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments. English required; MI license essential.

Qualifications

• Licensed Nurse Practitioner in Michigan with ANCC or equivalent certification.
• Minimum 2+ years NP experience with complex adult populations.
• Ability to practice independently in a startup environment.

Responsibilities

• Conduct initial and follow-up assessments for flagged residents.
• Initiate and adjust care plans; complete consent and coordinate with providers.
• Train staff on Circadia integration at the point of care.
• Document notes and charts within the same day.
• Cover sites within a 30-40 mile radius; mileage covered per policy.
