Job summary

Circadia Health is seeking an on-site Nurse Practitioner in Michigan to lead direct patient assessments and integrate AI-driven insights into care workflows. You will train facility staff, prioritize high-risk residents, and ensure timely interventions across multiple SNF sites in the Detroit area.

The role requires MI licensure, ANCC certification, and 2+ years in complex adult care, with strong independent practice and English proficiency.

Qualifications

• Certified and licensed Nurse Practitioner in MI.
• ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid MI driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

Responsibilities

• Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day
• High-volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
• Regional flexibility: Cover sites within a 30-40-mile radius; mileage and lodging covered per policy.
