Job summary

Circadia Health is seeking an on-site Nurse Practitioner to join its embedded care model. You will see patients, act on real-time AI signals, and coach facility teams to translate data into early interventions.

This full-time role requires Monday–Friday on-site presence at a partner facility in Montebello, CA, with regional travel within a 30–40 mile radius and mileage covered per policy.

Qualifications

• CA-licensed NP with ANCC or equivalent.
• Eligible for Medicare participation, company bills on your behalf.
• At least 2 years NP experience with complex adult populations.
• Valid CA driver’s license; able to travel locally.
• Thrives in a dynamic startup environment.
• English required.

Responsibilities

• Direct patient assessments and care plans.
• Train staff to integrate Circadia into clinical workflows.
• Interpret AI insights and drive timely interventions.
• Complete documentation on the same day.
• Conduct high-volume rounds at SNFs (≈25+ visits/day).
• Provide feedback to product and ops teams on field usage.
• Mentor NP onboarding and collaboration with clinical ops/product teams.
