Job summary

Circadia Health is hiring an on-site Nurse Practitioner to evaluate residents, respond to predictive signals, and coach facility teams. You will blend direct patient care with hands-on training and real-time AI-driven insights to improve care delivery.

You will collaborate with clinical ops/product teams, cover local sites within a 30–40 mile radius, and play a key role in reducing preventable readmissions through data-informed decisions.

Qualifications

• Licensed NP in AZ (ANCC or equivalent).
• Eligible for Medicare participation.
• 2+ years NP experience with complex adult populations.
• Valid AZ driver’s license; able to travel locally.
• Thrives in a dynamic startup environment.
• English required.

Responsibilities

• Direct patient care: Assess residents, initiate/adjust plans of care, obtain consent, coordinate with facility providers.
• Tech → bedside integration: Train staff to standardize Circadia in clinical workflows.
• Data into action: Interpret insights, prioritize high-risk patients, drive timely interventions.
