Job Overview
The Nurse Practitioner in Advanced Illness Management (AIM) delivers personalized palliative care, ensuring comprehensive treatment plans and demonstrating excellent clinical and ethical judgment. Responsibilities include diagnosis, treatment, and follow-up under physician supervision.

Key Responsibilities

1. Collaborate with physicians and team members to manage patient care, including diagnosis, treatment, and follow-up.
2. Conduct assessments covering disease progression, functional decline, symptoms, and impact on quality of life.
3. Develop and implement individualized care plans in consultation with patients, families, and the care team.
4. Manage medication prescribing, diagnostic testing, and treatments within legal scope.
5. Maintain ongoing communication with physicians and document care in medical records.
6. Participate in on-call duties as required and ensure effective team communication.

Qualifications

• Master's of Science in Nursing or equivalent, with valid Nurse Practitioner license.
• Minimum two years of NP experience, preferably in Family or Adult Medicine and Value-Based Care.
• Current licenses, certifications, and agreements as specified, including DEA licenses and certification in Hospice and Palliative Care preferred.
