Role Overview

The Nurse Practitioner in Advanced Illness Management (AIM) delivers personalized palliative care, ensuring comprehensive treatment plans and demonstrating excellent clinical and ethical judgment. Responsibilities include diagnosis, treatment, and follow-up under physician supervision.

Key Responsibilities

• Collaborate with physicians and team members to manage patient care from assessment to discharge.
• Develop individualized care plans, including symptom management and treatment adjustments.
• Conduct assessments, prescribe medications, order diagnostics, and coordinate care to meet patient needs.
• Participate in on-call duties, communicate with the healthcare team, and document clinical findings accurately.

Qualifications

• Master's of Science in Nursing or equivalent, with at least two years of Nurse Practitioner experience.
• Current licensure and certification as a Nurse Practitioner, with appropriate state licenses and DEA registration.
• Knowledge of palliative medicine principles, Medicare/Medicaid regulations, and interdisciplinary care processes.
