Nurse Practitioner - Advanced Illness Management

Illumia Health, a growing part of Gentiva, specializes in hospice and palliative care, partnering with physicians to provide advanced illness management and relief from symptoms and stress, enhancing quality of life for patients at home. Overview

As a Nurse Practitioner in Advanced Illness Management, you will develop comprehensive, individualized treatment plans, providing diagnosis, treatment, and follow-up under the guidance of the Palliative Medical Director or Primary Care physician. Your role is vital in delivering high-touch palliative care alongside curative treatments, focusing on improving patient quality of life.

Responsibilities
  • Collaborate with physicians and multidisciplinary team members to deliver advanced illness management and palliative care.
  • Manage patient care from initial assessment to discharge, considering disease progression, symptoms, and impact on quality of life.
  • Develop and implement personalized care plans in consultation with patients and families.
  • Coordinate care and facilitate access to necessary services.
  • Participate in on-call duties during evenings and weekends.
  • Prescribe medications, order and interpret diagnostic tests, and manage treatments within legal scope.
  • Maintain clear communication and documentation of clinical findings and care plans.
  • Engage with the care team to optimize patient outcomes and ensure quality measures are met.
Qualifications
  • Master’s of Science in Nursing or equivalent.
  • Minimum two years of Nurse Practitioner experience, with preferred specialization in Family or Adult Medicine.
  • Valid Nurse Practitioner license and certifications, including DEA registration.
  • Knowledge of palliative medicine principles, Medicare/Medicaid regulations, and interdisciplinary care processes.
  • Strong communication, decision-making, and autonomous working skills.
