Premise Health Inc. is seeking a full-time Nurse Practitioner to provide care for client employees at our Health Center in San Jose, CA. Schedule is 8:00am–5:00pm, Monday–Friday. The role includes dispensing responsibilities and a 103 will be required.

You will conduct exams, manage care plans, and collaborate with physicians to ensure high-quality, coordinated care. Premise offers comprehensive benefits and a strong growth path.

Competenze

• RN license required to practice in state
• Nurse Practitioner license required to practice
• Prescriptive Authority where applicable
• Federal DEA license (as per state requirements)
• Current national certification (ANCC or AANP)
• MSN or Higher; transcripts if certificate doesn't state nursing
• BLS/ACLS for healthcare providers required; ACLS may be required by contract scope
• At least three years of NP clinical experience in ambulatory care, occupational health, family practice, or ED

As a Full-Time Nurse Practitioner, you'll provide care to client employees in our Health Center located in San Jose, CA. The scheduled hours are 8:00am-5:00pm, Monday-Friday. This role will be providing dispensing and a 103 will be required. Join us and be a part of our growth!

What You’ll Do

• Conducts physical exams and preventive health measures per guidelines
• Performs procedures such as injections, suturing, wound care, and infection management
• Develops patient care plans, records progress, and ensures continuity of care
• Educates patients on treatment plans, health maintenance, and development
• Orders and interprets diagnostic tests to assess clinical conditions
• Collaborates with physicians and healthcare teams for comprehensive care
• Prescribes or recommends treatments, including medications and therapies
• Refers patients to specialists or physicians as needed
• Maintains patient health records ensure accurate and up-to-date documentation and records
• Educates and coaches nursing staff on best practices
• Performs other duties as assigned

What You’ll Bring

• Maintains state RN licenses/certificates to practice
• Maintains Nurse Practitioner licenses/certificates to practice
• Maintains State Prescriptive Authority (if applicable)
• Federal DEA license (matching each state of practice as applicable)
• Current national certification (ANCC or AANP required)
• Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)
• Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
• At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting

Premise provides its reasonable and genuinely expected range of compensation for this job of $168,064.00 - $210,080.00 annually. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.
