Job summary

Premise Health Inc. is seeking a Full-Time Nurse Practitioner to provide high-quality primary and preventive care to employees at a premier client health center in New York, NY near Central Park.

The role offers a Monday–Friday, 8:00 AM to 5:00 PM schedule, collaborative care, and a comprehensive benefits package. You will work with a team focused on exceptional clinical care and wellness.

Qualifications

• Maintain state RN licenses to practice.
• Maintain NP licenses to practice.
• Maintain Prescriptive Authority.
• Federal DEA license.
• State Narcotic/Controlled Substance license as required.
• National certification (ANCC or AANP).
• MSN degree.
• AT least BLS/ACLS certification as required.
• At least three years of clinical NP experience in ambulatory/occupational health/family practice/ER.

Responsibilities

• Conduct physical exams and preventive health measures per guidelines.
• Performs procedures such as injections, suturing, wound care, infection management.
• Develops patient care plans and ensures continuity of care.
• Educates patients on treatment plans and health maintenance.
• Orders and interprets diagnostic tests.
• Collaborates with physicians and teams for comprehensive care.
• Prescribes or recommends treatments and therapies.
• Refer patients to specialists as needed.
• Maintain accurate and up-to-date patient records.
• Educate and coach nursing staff on best practices.
• Perform other duties as assigned.

As a Full-Time Nurse Practitioner, you will provide high-quality primary and preventive care to employees of a premier client at our health center in New York, NY, conveniently located near Central Park. This position offers a consistent Monday through Friday schedule from 8:00 AM to 5:00 PM, allowing you to build meaningful patient relationships while maintaining a healthy work-life balance. The ideal candidate will have strong clinical skills and a strong emergency response background, with the ability to confidently assess and respond to urgent medical situations. You’ll work as part of a collaborative care team focused on delivering exceptional healthcare and promoting overall wellness.

What You’ll Do

• Conduct physical exams and preventive health measures per guidelines

• Develops patient care plans, records progress, and ensures continuity of care
• Educates patients on treatment plans, health maintenance, and development
• Orders and interprets diagnostic tests to assess clinical conditions
• Collaborates with physicians and healthcare teams for comprehensive care
• Prescribes or recommends treatments, including medications and therapies
• Refers patients to specialists or physicians as needed
• Maintains patient health records ensure accurate and up-to-date documentation and records
• Educates and coaches nursing staff on best practices
• Performs other duties as assigned

What You’ll Bring

• Maintains state RN licenses/certificates to practice
• Maintains Nurse Practitioner licenses/certificates to practice
• Maintains State Prescriptive Authority (if applicable)
• Federal DEA license (matching each state of practice as applicable)
• State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration)
• Current national certification (ANCC or AANP required)
• Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)
• Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
• At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting

Premise provides its reasonable and genuinely expected range of compensation for this job of $155,000.00 - $165,000.00 annually. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.
