Job summary

Premise Health Inc. is seeking a Full Time Nurse Practitioner at our Health Center in Scottsdale, AZ. The role involves conducting exams, improving preventive care, and collaborating with physicians to deliver comprehensive care.

You will manage patient care plans, order tests, educate patients, and maintain accurate records. A valid NP license, prescriptive authority, and national certification are required, along with BLS; ACLS may be needed per contract.

Qualifications

• Maintain state RN licenses/certificates to practice.
• Maintain Nurse Practitioner licenses/certificates to practice.
• Maintain State Prescriptive Authority (if applicable).
• Hold a Federal DEA license (matching each state of practice).
• Possess current national certification (ANCC or AANP).
• Master’s Degree in Science of Nursing required.
• Hands-on BLS certification; ACLS may be required per contract.

Responsibilities

• Conduct physical exams and preventive health measures per guidelines.
• Perform procedures such as injections, suturing, wound care, and infection management.
• Develop patient care plans and ensure continuity of care.
• Educate patients on treatment plans and health maintenance.
• Order and interpret diagnostic tests to assess conditions.
• Collaborate with physicians and healthcare teams for comprehensive care.
• Prescribe or recommend treatments, including medications and therapies.
• Refer patients to specialists as needed.
• Maintain up-to-date patient health records.
• Educate and coach nursing staff on best practices.

As a Full Time Nurse Practitioner, you'll provide care to client employees in our Health Center located in Scottsdale, AZ.

Schedule:

• 32 hours per week
• Tuesday - Friday, 7:00a - 4:00p
• No NIghts, No Weekends, No Holidays!

What You’ll Do

• Conducts physical exams and preventive health measures per guidelines
• Performs procedures such as injections, suturing, wound care, and infection management
• Develops patient care plans, records progress, and ensures continuity of care
• Educates patients on treatment plans, health maintenance, and development
• Orders and interprets diagnostic tests to assess clinical conditions
• Collaborates with physicians and healthcare teams for comprehensive care
• Prescribes or recommends treatments, including medications and therapies
• Refers patients to specialists or physicians as needed
• Maintains patient health records ensure accurate and up-to-date documentation and records
• Educates and coaches nursing staff on best practices
• Performs other duties as assigned

What You’ll Bring

• State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration)
• Current national certification (ANCC or AANP required)
• Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)
• Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
• At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting
