Job summary

True North Consultants, Inc. is seeking a full-time Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide comprehensive primary care in the Greater Sacramento area. You will collaborate with physicians and care teams to serve underserved populations within a community health center.

Compensation ranges from $131,000 to $170,000 annually, with relocation assistance available. Typical schedule is Monday–Friday 8–5 or a 4-day 10-hour shift option, across multiple

Qualifications

• Active CA RN and NP license.
• Current NP board certification.
• Current DEA certification.
• Current BLS certification.
• 1–2 years of clinical experience preferred, especially in community health or underserved populations.
• Strong commitment to serving low-income, homeless, or medically underserved communities.
• Comfortable treating three patients per hour.
• EPIC experience.
• In-basket management competency.
• Bilingual skills in English and Russian, Farsi, Pashto/Pushto, or Spanish strongly preferred.

Responsibilities

• Perform comprehensive patient evaluations, including history and physical examinations.
• Diagnose and manage acute and chronic medical conditions.
• Order and interpret laboratory tests, imaging, and other diagnostics.
• Prescribe medications and therapeutic interventions within scope of practice.
• Develop and implement treatment plans in collaboration with the Medical Director.
• Monitor patient progress and modify care plans as appropriate.
• Initiate emergency procedures when clinically indicated.
• Provide preventive care, health education, and counseling services.
• Maintain accurate and timely documentation in accordance with agency and regulatory standards.
• Collaborate with physicians and other healthcare professionals to ensure coordinated, high-quality care.

Job Type: Full-time, Mondays to Fridays 8-5 PM or 4 day (10 hour shift available).

Location: Greater Sacramento area, CA (Multiple locations available)
