Job summary

Liberty Hospice is seeking a part-time Nurse Practitioner to join its hospice and palliative care team. The role offers 2-3 days per week with responsibilities to implement program requirements, perform patient visits, and coordinate care with the Palliative Care team.

The NP must hold a Master’s in Nursing, current licensure, and Adult or Family NP certification, with preferred hospice experience. Strong communication and organizational skills are essential.

Qualifications

• Master’s degree in Nursing with active state licensure.
• Certification as Adult or Family Nurse Practitioner.
• Hospice or Palliative Care nursing experience preferred.

Responsibilities

• Implement palliative care program requirements.
• Perform patient visits per guidelines and coordinate with the Palliative Care team.
• Conduct face-to-face recertification encounters for Hospice patients with Medical Directors.
• Provide clinical findings to PCP and/or supervising Medical Directors.
• Communicate with patients and families at all levels.
• Coordinate communication with agency personnel.

HOSPICE & PALLIATIVE CARE NURSE PRACTITIONER

Part-Time, 2-3 days/week

Job Description:

• Implements palliative care program requirements
• Completes medical visit with patients per guidelines and coordinate care with Palliative Care team as clinically indicated
• Conduct face to face encounter to provide re-certification information to Medical Director/Associate Medical Director for Hospice Patients
• Provide clinical findings of that face to face encounter Primary care physician, and/or supervising Medical Director/Associate Medical Director
• Communicate with patients/families at all levels.
• Communicate with all agency personnel

Job Requirements:

• Master’s degree in Nursing required, with current licensure in the state of practice
• Certification as an Adult or Family Nurse Practitioner
• Experience in Hospice or Palliative Care nursing preferred
• Knowledge of the framework, function, and organization of a hospice care program.
• Excellent communication and organization skills.
